UPM staff/student need to fill in the following form then submit it to the department general office :

 

• Borang Permohonan Menghadiri Seminar/Konferensi/Bengkel (Fail Borang Hadir Latihan) - (for Faculty of Science only)
• Borang Pelarasan Antara PTJ - for all UPM staff/student
• Borang Permohonan/Kebenaran untuk mengikuti latihan (Fail mohon latihan)- for all UPM staff
